Pertemps Social Care are recruiting social workers for roles within a Hospital Team in Luton.
You will be working as part of a multi-disciplinary team, assessing and facilitating safe and timely discharges with a commitment to promoting independence. You will work in a supportive team of Social workers and Social care support officers. The team works to tight deadlines, with a fast turnaround of work.
Your core responsibilities will include:
- Rapid Assessment under Discharge to Assess guidelines for hospital discharge
- Assessing service users, mental capacity and making best interest decisions
- Liaising with the discharge liaison team, as well as ward staff and family members.
- Arranging care packages on discharge and step down beds.
- Completing reviews and support plans.
- Reporting any delays with service users.
