Job Description
You will be:
Joining the Transition and LD Adult Team as a Case Manager; your role will be to:
- Undertake back logs of reviews to ensure cases are properly reviewed.
- Carry out a range of Social Care tasks including screening and triage, initial assessment, support planning and case coordination.
- Working as part of a multi – disciplinary team of Social Workers/ Occupational therapists, case managers and support workers/advisers.
- Right to work in the UK.
- Qualification credit Framework level 3 (QCF) in health and social care.
- Experience of working with adults in Social Care/Health/Voluntary Community Sector setting.
- A Dedicated Consultant specialising in their Sector to ensure candidate satisfaction and matching you to the right role.
- A smooth compliance and payroll system.
