Job Description
You will be:
Joining the Complex Care Team; your role will be to:
- Managing complex case loads of Service Users with Learning/ Physical Disabilities/ Mental Health and Older Adults.
- Collaborating with other healthcare professionals to ensure comprehensive care.
- Addressing any urgent or complex situations, ensuring the appropriate care plans and assessments are carried out.
- Maintaining an accurate records of patient interactions.
- Registered to Social Work England and has completed an ASYE.
- The ideal Candidate will have direct experience of working within a hospital setting within Adult Social Work.
- Right to work in the UK.
- A Dedicated Consultant specialising in their Sector to ensure candidate satisfaction and matching you to the right role.
- A smooth compliance and payroll system.
