Adult Social Worker - Hospital Team

• £34.35 - £36 per hour
• Barrow in Furness
• Full time, condensed hours available
• 14 month contract

About the role:
This post is  for an experienced social worker  who will  be based across 2 sites which deliver intermediate care on discharge from   hospital. The IC beds are based  Park View gardens in Barrow in Furness and Maudes Meadow in Kendal.

Intermediate care beds following discharge from hospital are a short‑term, time‑limited service designed to support people who are medically fit to leave hospital but are not yet able to return home safely.
Progress in intermediate care beds following hospital discharge is monitored in a structured, multidisciplinary, and outcome‑focused way, with regular reviews against agreed recovery goals.
EPW Social worker is required  as  part of  the wider  MDT to  monitor and gave oversight of the pathway against clear, time‑limited goals, agreed with the person (and carers where appropriate). Ie

• Regaining independence (e.g. walking, washing, dressing)
• Improving strength, balance, or endurance
• Managing medication or health conditions safely
• Preparing for discharge home

The role of the SW is to support timely, safe discharge form the IC unit, promote independence and ensure that long‑term needs are appropriately assessed or make a determination for reablement .
Social workers  will play a part in oversee assessment, decision‑making, risk management, and onward planning and will be part of the Hospital discharge team.


What you need:

• A degree in Social Work or equivalent
• Registered with Social Work England
• Completed relevant post-qualifying training
• Enhanced DBS check
• Excellent communication and written skills
