Children's Social Worker - 14+ Team
All Wales Pledge rate
South Wales
37 hours p/w, hybrid working

About the role:
To provide a high-quality social work service to young people aged 14 and above who are preparing to leave care or are already in the process of transition to adulthood. The postholder will promote positive outcomes in education, health, emotional wellbeing, and independence, ensuring that young people are supported in line with the Social Services and Well-being (Wales) Act 2014 and associated legislation and guidance.

Key Responsibilities
  • Hold a caseload of young people aged 14+ who are looked after or care leavers, providing statutory support and interventions in accordance with national legislation and local policy.
  • Undertake assessments, care and pathway planning, and reviews to ensure that young people have clear, outcome-focused plans for their transition to adulthood.
  • Build and maintain trusting relationships with young people, promoting participation in decision-making and planning for their future.
  • Work collaboratively with families, carers, schools, health professionals, and partner agencies to deliver coordinated, person-centred support.
  • Prepare reports for case conferences, reviews, court proceedings, and other formal meetings, ensuring that all recording is timely and of a high standard.
  • Support young people to access appropriate education, training, employment, and accommodation opportunities.
  • Safeguard and promote the welfare of young people in accordance with safeguarding procedures and thresholds.
  • Contribute to the development of service delivery, policy, and best practice within the 14+ team and the wider Children’s Services department.
  • Participate in regular supervision, reflective practice, and professional development opportunities in line with registration requirements with Social Care Wales.
What you need:
  • A degree in Social Work or equivalent
  • Registered with Social Care Wales/ Social Work England
  • At least 3 years PQE direct employment within English Local Authorities
  • Completed relevant post-qualifying training
  • Enhanced DBS check
  • Excellent communication and written skills
