About the role:
To complete statutory social work service to children and young people with disabilities and their families. The post holder will undertake assessments, develop care and support plans, and ensure the safeguarding and wellbeing of children in accordance with legislative requirements, statutory guidance, and organisational policies.
Key Duties and Responsibilities;
- Assessment and Care Planning
- Undertake comprehensive assessments of children and young people with disabilities, considering their social, emotional, educational, and health needs.
- Formulate, implement, and review care and support plans in partnership with children, families, and relevant professionals.
- Ensure plans are outcome-focused, strengths-based, and compliant with statutory requirements.
- Safeguarding
- Identify and respond to safeguarding concerns in line with statutory frameworks and organisational procedures.
- Participate in child protection enquiries, case conferences, core groups, and legal proceedings as required.
- Ensure the safety and welfare of children remain paramount in all decision-making.
- Direct Work with Children and Families
- Establish and maintain effective relationships with children, young people, and their families.
- Provide practical and emotional support to promote resilience and independence.
- Advocate for the rights of children with disabilities to access education, leisure, and community opportunities.
- Multi-Agency Working
- Collaborate with health, education, voluntary sector, and other professionals to deliver coordinated services.
- Contribute to Education, Health and Care Plans (EHCPs) and transition planning into adulthood.
- Case Management and Recording
- Manage a caseload in accordance with service priorities and statutory timescales.
- Maintain accurate, timely, and comprehensive case records using electronic case management systems.
- Prepare high-quality reports for statutory reviews, panels, and court proceedings
What you need:- A degree in Social Work or equivalent
- Registered with Social Work England
- At least 3 years PQE direct employment within English Local Authorities
- Completed relevant post-qualifying training
- Enhanced DBS check
- Excellent communication and written skills
