Are you an experienced and passionate Social Worker with a background in hospital discharge? We’re working with a local authority in West Yorkshire seeking skilled Qualified Social Workers to join their Hospital Discharge and Home First Team. This is a fantastic opportunity to support people transitioning from hospital to home, while working in a fast-paced, multidisciplinary environment.

🏥 About the Role
As part of the Hospital Discharge Team, you’ll help facilitate timely, safe discharges from acute settings — ensuring the right care and support is in place from day one.
You will:
- Manage a caseload of adults with complex needs
- Carry out strengths-based assessments and create tailored care and support plans
- Deliver statutory responsibilities, including Care Act assessments, reviews, and safeguarding interventions
- Work closely with health professionals, therapy teams, discharge coordinators, and care providers
- Prioritise early intervention, recovery, and independence
- Participate in a duty rota (1 week in every 4)
🧠 About You
To be successful in this role, you’ll need:
✔️ A recognised Social Work qualification (BSc, BA, MA, DipSW, CQSW)
✔️ Registration with Social Work England
✔️ Hospital discharge experience or equivalent acute-based adult services background
✔️ In-depth understanding of the Care Act, Mental Capacity Act, and safeguarding frameworks
✔️ Experience of managing a complex caseload and working under pressure
✔️ Strong communication, assessment, and decision-making skills
✔️ A commitment to person-centred, rights-based practice
