If so, Derbyshire County Council Adult Social Care and Health wants to hear from you. We are seeking to recruit enthusiastic and motivated Social Care Practitioners for a short-term contract until March 2026, covering the High Peak and Derbyshire Dales. Face to face visit in the community are an integral element of this role.
The nature of the work will be to support a backlog of non-complex Occupational Therapy waiting list assessments. Candidates do not need to be a qualified Occupational Therapist but should have a Level 3 Diploma in Adult Care.
Candidates will need strong assessment skills which are strengths based. The post holder will be working closely with the person's own assets as well as with an equipment provider and Housing and Health departments to support people to remain independent at home through small aids and minor and major adaptations. Candidates will need a strong track record of working in a local authority assessing and accurately recording need.
You must have a proven track record of working successfully and efficiently with adults in the community in a Local Authority environment and have previous experience of Occupational Therapy interventions. This may include equipment solutions, minor and major adaptations, including partnership working with housing providers.
We have an experienced management team who are committed to providing good support, advice, supervision, and a welcoming team ethos.
You will be supported in your role by a wider network of Social Care Practitioners, Occupational Therapists, Team Coordinators including a dedicated Occupational Therapy service manager and dedicated Occupational Therapy senior practitioners.
As part of our Adult Care Team, you will be required to take a strength-based approach to deliver a proportionate, personalised, safe and sustainable response for local adults, their carers, and families.
A positive approach to risk taking, accurate and timely record keeping, personal resilience and the ability to contribute to inter agency and multi-disciplinary approaches both across the council and in other statutory bodies are key to this role.

• Have knowledge and/or experience of working within a person centred, holistic and strengths-based way.
• Have knowledge and/or experience in assessing a person’s activities of daily living.
• Have knowledge and/or experience of physical, learning disabilities and/or mental health conditions and the impact on activities of daily living.
• Have knowledge and/or experience of equipment and adaptation solutions.
• Have the ability to manage and prioritise your casework.
• Have good communication skills and the ability to form positive relationships with a wide range of partners, professionals and agencies.
• Have good IT skills, time management skills and be highly motivated. Experience of Mosaic recording system is desirable.
• Have an understanding of statutory legislation that underpins Occupational Therapy assessment and intervention in adult social care and health.
• Be a reflective practitioner with a commitment to supervision and development working within relevant codes of conduct.
• Have knowledge and/or experience of safeguarding principles and how these are embedded in everyday practice.
• Ability to travel for home visits.

Duties & Reponsibilties:
Work with clients and in association with their families/carers to identify and mobilise their strengths, networks, communities and help to maximise independence, self-care, social and occupational opportunities as appropriate.
Working in conjunction with other professionals including social workers and occupational therapists, referring on as necessary.
Responsible for the routine assessment and review of individual needs and arranging, as appropriate, the provision or referral of services; to include prescribing and delivering a range of simple equipment and non-complex housing adaptations in line with preventative approaches.
Involved jointly with the senior practitioner/service manager, reviewing work and caseload at regular intervals, including the closure and transfer of cases;
Undertake holistic assessments for individuals with less complex needs, in order to identify eligible unmet outcomes and assist clients to create viable care and support plans in keeping with the relevant legislation;
Disseminating information to clients as required such as resources available, including linking people to local mainstream community resources/activities as appropriate;
Liaison with voluntary bodies, residential and day establishments and other agencies or groups working in the area, as appropriate;
Ensure the safeguarding of vulnerable adults.
