Qualified Social Work Team Manager to lead a dedicated
Hospital Discharge Team in West Yorkshire. You’ll be at the heart of operations, driving quality practice, managing complex casework, and supporting your team through high-pressure frontline work.

Your Role Will Include:- Leading and supporting a team of Social Workers and practitioners delivering hospital discharge planning and safeguarding work.
- Overseeing Care Act assessments, capacity decisions, and case management.
- Managing a duty function within the team and ensuring timely, effective responses to safeguarding concerns.
- Working closely with NHS, health professionals, and partner agencies to ensure seamless multi-agency working.
- Ensuring your team’s compliance with relevant legislation including the Care Act 2014, Mental Capacity Act, Human Rights Act, and Mental Health Act.
- Maintaining a strong presence across hospital sites and local offices to support staff and ensure operational excellence.

What We're Looking For:- Qualified Social Worker, registered with Social Work England.
- Proven leadership/management experience in adults' services—especially within hospital discharge or safeguarding teams.
- Excellent understanding of current adult social care legislation and statutory responsibilities.
- Strong people management and supervision skills, with the ability to motivate and support team members.
- Confidence managing complex cases and risk in a fast-paced environment.
- A commitment to high-quality, person-centred care and positive outcomes for vulnerable adults.
