Are you a passionate and experienced Social Worker ready to make a real impact in Adult Social Care? We're looking for someone just like you to join a dynamic Hospital Discharge to Assess Team in Greater Manchester. This is more than just a role — it's a chance to be part of a supportive, multi-disciplinary team that puts citizens at the heart of everything they do.
✅ What You’ll Be Doing:
- Leading on assessments, care planning and discharge coordination
- Making sound decisions using your professional knowledge and experience
- Working closely with NHS colleagues, therapists, providers and families
- Delivering safe, person-centred care in line with legislation and best practice
- Managing a caseload while ensuring timely, effective interventions
- Supporting less experienced staff and students
👤 What We’re Looking For:
- Social Work England registered
- Degree (or equivalent) in Social Work
- 3+ years' experience in Adult Social Care (Hospital/Discharge experience ideal)
- Skilled in assessments, care planning and risk management
- Confident working in a fast-paced, multi-agency environment
- Passionate about delivering the best outcomes for vulnerable adults
- Strong communication, mediation and relationship-building skills
