Are you passionate about making a positive impact in Adult Social Care? We’re seeking a dedicated and experienced Qualified Social Worker to join a dynamic Adults team in North Wales. In this key role, you’ll support adults with complex needs, promoting independence, safeguarding, and enhancing quality of life.  You'll manage complex cases, screen safeguarding referrals, and work under key legislative frameworks including the Care Act 2014, Mental Capacity Act 2005, and Mental Health Act 1983. This is an exciting opportunity to make a real difference in the lives of vulnerable adults and their families.

🔍 Your Role Will Involve:
• Managing a caseload of adults with complex needs, including safeguarding and care planning
• Supporting individuals to live as independently as possible, including developing life skills and personal welfare
• Conducting Care Act assessments and coordinating multi-agency interventions
• Screening and triaging safeguarding referrals within a supportive team environment
• Engaging in regular duty work, planning care pathways, and protecting individual rights
• Applying up-to-date knowledge of legislation and safeguarding procedures to support best outcomes

🧠 What We’re Looking For:
• Qualified Social Worker registered with Social Work England
• Minimum 4 years' post-qualified experience in Adult Social Care
• Strong understanding of the Care Act, Mental Capacity Act, Human Rights Act, and related legislation
• Experience working with adults with complex needs, learning disabilities, or mental health challenges
• A person-centred, strength-based approach with excellent communication and advocacy skills
• Confidence working as part of a multi-disciplinary team to plan and coordinate care
• Passionate about improving lives and striving for the best possible outcomes
