Are you passionate about empowering young people as they move into adulthood? We’re seeking a dedicated and experienced Qualified Social Worker to join a forward-thinking Transitions Team (16–25) in North Wales. In this key role, you’ll support young people with complex needs as they transition from Children’s Services to Adult Social Care—helping them build independence, confidence, and brighter futures. You'll manage complex cases, screen safeguarding referrals, and work under key legislative frameworks including the Care Act 2014, Mental Capacity Act 2005, and Mental Health Act 1983. This is an exciting opportunity to make a real difference in the lives of vulnerable young people and their families.
🔍 Your Role Will Involve:- Managing a caseload of young people aged 16–25, including complex safeguarding and transitions work
- Supporting the shift toward independent living, including life skills development, personal welfare, and decision-making
- Conducting Care Act assessments and coordinating multi-agency interventions
- Screening and triaging safeguarding referrals within a supportive team environment
- Engaging in regular duty work, planning care pathways and protecting individual rights
- Applying up-to-date knowledge of legislation and safeguarding procedures to support best outcomes
🧠 What We’re Looking For:- Qualified Social Worker registered with Social Work England
- Minimum 4 years' post-qualified experience in Adult Social Care or Transitions
- Strong understanding of the Care Act, Mental Capacity Act, Human Rights Act, and related legislation
- Experience working with complex needs, learning disabilities, or vulnerable young adults
- A person-centred, strength-based approach with excellent communication and advocacy skills
- Confidence working as part of a multi-disciplinary team to plan and coordinate care
- Passionate about improving lives and striving for the best possible outcomes
