Our client is looking for Qualified Adult Social Workers to join the Hospital Discharge team. We are looking for experienced and passionate Social Workers to join a dedicated Hospital Discharge team
. All candidates must have previous experience in working in Adult Social Care and have a good understanding and be able to apply appropriate legislation. Knowledge is required of the Care Act 2014, Mental Capacity Act 2005, Human Rights Act, Mental Health Act, etc. This team is responsible for screening incoming safeguarding referrals as well as case holding both complex safeguarding cases. Successful candidates will be expected to work in a specific area within The Authority and responsible for carrying out Care Act assessments, Safeguarding work, multi-agency work, as well as carrying out a duty function within the team. This role is fully office based role.
In this role you will be accountable for the day to day management of a team of Social Workers and other professionals in the delivery of statutory and legislative duties and requirements. You will manage the team to safeguard and promote the welfare of vulnerable children and young people in this local authority whilst effectively managing risk. Ensuring the key performance targets as they apply to the team, the service and the Council are met. Carrying out this role in a collaborative manner that promotes equality of opportunity and joint working with other teams across the Council and partners, while challenging and supporting the team to ensure timely and suitable outcomes for child, young people and their families and embedding the Council’s values into the work of the team and peers. You will take a leading role in promoting, delivering and embracing the local authority child care policies and help deliver the vision of making children safe, secure and successful. Use all multi-disciplinary resources available to ensure that the quality of practice with children, young people and families is of a consistently high quality, is cost effective and that quality service standards are met.
Working in accordance with social care legislation and local government requirements, informed by the values and principles of social care; you will be supporting in the delivery of person centred, safe and effective social care services. Social Work experience and excellent communication skills you’ll establish and maintain relationships with service users and other agencies to ensure services meet identified needs.

Perfect candidate will be:- Qualified Social Worker registered with Social Work England .
- SW, CSS, DipSW or Social Work Degree Creative, flexible and forward thinking in approach.
- Work in a Social Services Directorate or other Statutory or Voluntary Agency.
- Constantly striving for continuous improvement to provide the best outcomes for individuals and their families
- Able to use excellent mediation and negotiation skills on behalf of service users.
- Work with a range of client groups within an Agency
- Highly skilled at building meaningful relationships with children and families to engage them in assessments and plans
- Committed to providing our service users with a high-quality service
- Experienced in working in a multi-disciplinary, interagency basis
- Knowledge of legislation relating to general social work practice
- 4 Years plus Qualified
In this role you will:- Use skills, knowledge, and experience to make full assessment of risks protection and social care needs.
- Manage an agreed case load.
- Use specialist communication skills to achieve optimum outcomes for service users.
- Devise individual care plans, taking account of diversity and promoting dignity and empowerment.
- Offer advice and support to unqualified staff and students to assist development.
