Adult Social Worker - Hospital Assessment Team
£38.76 per hour 
South West wales
38 hours p/w, hybrid

The role for an agency worker has become available to join a committed and motivated Hospital Assessment Team who work hard to deliver excellent standards in a positive working environment. The Hospital Team works in partnership with individuals/ multi professionals to identify what matters to them. This includes undertaking Mental Capacity Assessments, Continuing Health Care Assessments, leading Best Interest Meetings to ensure people have a safe and timely discharge from hospital.

You will work alongside a range of professionals within the teams, including colleagues from the health board, therapies, external providers and families. It is an exciting time to join adult social care with friendly, experienced and supportive colleagues who are dedicated to providing a service that puts citizens at centre of their work. 

To be considered for this post you must be registered with Social Care Wales, at least 3 years post qualifying experience as a social worker and have not left a Local Authority permanent contract within Wales in the last 6 months.
 
What you need:

• A degree in Social Work or equivalent
• Registered with Social Care Wales/ Social Work England
• Completed relevant post-qualifying training
• Enhanced DBS check
• Excellent communication and written skills
