Job Description
As part of an Adults Locality Assessment team, you'll play a crucial role in providing tailored support and services to help individuals lead fulfilling lives.
You will;
- Conduct comprehensive assessments of adults' needs and circumstances.
- Develop and implement person-centred care plans to support individuals in achieving their goals.
- Work collaboratively with health professionals, care providers, and other agencies to ensure integrated care.
- Provide guidance, support, and advocacy to adults and their families.
- Monitor and review the effectiveness of care plans, making adjustments as necessary.
- Maintain accurate records and prepare reports in line with statutory requirements.
