Education Health Care Plan Co-ordinator
Location: Greater London
Pay rate: £201.34 per day

Our Client is looking for an experienced and passionate Education Health Care Plan Co-ordinator for a Local Authority in the South East.

Main Functions of Post
Responsible for a defined caseload and act as the named officer in line with the Special Educational Needs (SEN) Code of Practice, statutory responsibilities and the authority’s local policies and strategies. Responsible for the management of a designated SEN caseload, ensuring service standards, performance outcomes and statutory requirements are met. Be the primary point of contact for the parents/carers of children and for young people, across the 0 - 25 age range who are undergoing an Education, Health and Care (EHC) Needs Assessment or who have an Education, Health and Care plan.

Key responsibilities

To develop and maintain professional, supportive working relationships with key staff in schools and other services in the locality.
To ensure all written correspondence and documents are accurate and well structured, reflecting local and national requirements, including SEN legislation, and is sensitive to the perspective of the recipient.
To ensure electronic case files are well maintained to agreed quality standards and confidentiality in line with Data Protection Act. To accurately record, input and maintain records on a database as required. To attend a range of meetings with young people, parents, school staff and other officers and professionals as required, including annual reviews in line with agreed priorities.
To support the SEN Team Manager and Case Officers in the preparation of information for management reports, tribunal case statements, and general correspondence as requested.
To prioritise and manage workloads to ensure all transfer reviews are managed within agreed timescales.
To understand the SEND Code of Practice and work within the legislative framework. Safeguarding is everyone's responsibility and all employees are required to act in such a way that at all times safeguards the health and well-being of children and vulnerable adults. Carry out duties with due regard to the Council’s Customer Care, Equal Opportunities, Information Governance, Data Protection, Safeguarding and Health and Safety policies and procedures. Undertake any other duties commensurate with the general level of responsibility of this post.
To chair any formal meetings as appropriate relating to the child's/young person’s assessment.
To ensure continued school/college liaison: - collecting, monitoring and acting on schools’ and colleges’ concerns and suggestions around communications, assessment decisions and timescales. To take responsibility for ensuring written responses are made to solicitors’ enquiries, liasing as appropriate with the LA’s own solicitor
To take responsibility for preparation of all relevant casework for Judicial Review, Special Education Needs Tribunal or other Court proceedings and to represent the Authority at such hearings as appropriate and be responsible for all decisions made. To contribute as appropriate to LA policy and procedures, strategic planning and review, including the development, monitoring and review of the SEN business plan.
To contribute to the organisation and delivery of training and conference events To be responsible for implementing the decisions of the SEN Panel including ensuring that the decisions of the Panel are conveyed to parents, carers, schools and other professionals - to be completed within statutory timescales.
To understand implement the Council's Equal Opportunities Policy in relation to the duties of the post, and be responsible for ensuring that caseworkers also follow policy. You may be developed and act as subject matter specialist for others across the team; examples of specialism include; early years, post 16, electively educated children, autism, youth offending/prisons, children in care.

Special Condition

On occasion work in the evening or at weekends will be required for which time off in lieu arrangements will apply.
