Job Description
Pay rate: £36 Per Hour
Duties Include:
- To contribute operationally to the delivery of Newham Council’s Adult Social Care safeguarding and independent living strategies across ASC and Health commissioning areas, in partnership with locally commissioned specialist substance use service providers.
- To provide expert social work, case management, needs and risk assessment processes input when and where required.
- To flexibly support initial response to the social care needs of people with substance use issues taking in to consideration all safeguarding risks, including extra familial and contextual safeguarding indicators that may impact other individuals.
- To hold a case load of complex cases in respect of those individuals who are known to Change Grow Live (Local specialist commissioned substance use service) and who meet the threshold for ASC Services in line with the Care Act Section 9 outcomes 2014.
- To develop strong links with local service providers with a priority on: Substance use rough Sleeping, Street Outreach, Primary and Secondary Mental Health services, NCfMH, Praxis (No recourse to public funds) Local AMHP (Mental Health Local Authority) and local Enforcement and Police services
