Adults - Social Worker Disabilities
Location: South Powys, Mid Wales
Contract: 5th May 2025 – 31st July 2025 (12 Weeks)
Hours: 37 hours/week, Monday–Friday, 9:00 AM – 5:30 PM

What You’ll Do:
Assess the holistic needs of adults with disabilities and their carers, negotiating and reviewing care packages within the first six weeks to promote independence.
Manage a complex caseload, balancing risks while safeguarding vulnerable individuals and ensuring their wellbeing.
Collaborate with Reablement and other agencies to create seamless care pathways, strengthening multi-disciplinary working.
Undertake safeguarding investigations, Mental Capacity Act assessments, and Best Interests decisions to protect those at risk.
Promote social inclusion, equal opportunities, and diversity, while advocating for service users’ needs and resources.
What We’re Looking For:
Skills: Strong assessment, care management, and safeguarding skills, with knowledge of the Social Services and Wellbeing Act 2014.
Qualities: Compassionate, assertive, and able to manage conflict while advocating for vulnerable adults.
Experience: Proven experience as a Social Worker, ideally within adults’ disabilities services.
Requirements: Registered with Social Care Wales; Enhanced DBS check; ability to travel regularly into South Powys for office visits and client meetings.
Bonus Points: Welsh language skills (or willingness to learn basic greetings); experience with Mental Health Law or Best Interests assessments.
