Join Our Team as a Senior Social Work Practitioner! Are you a dedicated and experienced Social Worker with a passion for supporting adults with mental health needs? Network Recruitment Wales is seeking a Senior Social Work Practitioner to join the Adult Services Mental Health Team with one of our clients. This is an exciting opportunity to make a real difference in the community, delivering high-quality care and support to promote independence, dignity, and recovery.
About the Role: As a Senior Social Work Practitioner, you will work within a multi-disciplinary Community Mental Health Team, delivering services through the Care and Treatment Planning approach. You will assess individual needs, manage risks, and coordinate care plans to ensure positive outcomes for service users and their carers.
Key responsibilities include:
- Conducting assessments and delivering planned care to support community-based mental health services.
- Fulfilling the role of an Approved Mental Health Professional (AMHP) as required, adhering to the Mental Health Act and Code of Practice.
- Monitoring and evaluating care plans to achieve agreed outcomes and support recovery.
- Collaborating with stakeholders, including Aneurin Bevan University Health Board and third-sector agencies, to ensure integrated service provision.
- Supporting the development of less experienced staff and contributing to service development.
- Maintaining accurate casework records using systems such as SWIFT and Epex.
Essential Qualifications & Experience:
- Diploma or Degree in Social Work (or equivalent predecessor qualification).
- Registered as a Social Worker with Social Care Wales (or equivalent UK regulatory body).
- Minimum of 2 years’ post-qualifying experience in Adult Mental Health Social Work.
- Experience in Care and Treatment Planning and assessing/formulating risk management plans.
- Knowledge of the Social Services and Wellbeing Act 2014, Mental Health Act 1983, and related legislation.
Desirable:
- Approved Mental Health Professional (AMHP) qualification.
- Welsh language skills.
- Experience in AMHP practice.
Key Skills:
- Skilled in assessing needs and risks, devising creative care plans, and prioritising workloads.
- Strong communication and teamwork abilities, with the capacity to work autonomously.
- Proficient in ICT, including Microsoft Office, Outlook, and social work databases.
- Commitment to continuous professional development and equality of opportunity.
Additional Requirements:
- Valid UK driving licence
- Enhanced DBS check and registration with Social Care Wales.
- Flexibility to work across a 7-day rota as needed.
- Required documents: Criminal Convictions Declaration, Confidentiality Agreement, 5 years’ references/work history, Health Declaration/Questionnaire.
