Pay rate up to £35 per hour
The Talent Crowd are recruiting for Qualified Social Worker to join a Mental Health Community Team in Tameside.
 
This Social Work role will provide a comprehensive social care service to adults of working age who experience moderate to severe mental health problems within the community. The Social Worker will

• Work collaboratively across a range of disciplines to continuously promote a ‘needs based’ approach to all service users.
• Promote a partnership approach to service delivery.
• Develop clear support plans following a Care Act assessment, complete funding applications, complete work that has legal requirements such as Copdol, dols, 21a’s, reports for court etc
• Develop meaningful engagement with individuals who are often referred to as ‘difficult to engage’ and their families.
• Support Carers through the Care Act
• Work with Children about to turn 18 and transition into Adult Mental Health services
• The team will offer a 5-day service within the hours of 09.00 hrs to 16:30 hrs, the Monday to Friday 
• Based in the Tameside area
