Pay rate £30 per hour

Vitalis are recruiting for an experienced Social Worker to work in an Immediate Care Team in Bolton Council.
 
We are looking to recruit an enthusiastic and motivated individual who has a passion for working with adults, understanding their needs, aspirations and supporting them to enhance their quality of life and promote independence.
As a Social Worker in IMC you will work in one of the designated services and undertake assessments/reviews of need under the Care Act 2014, assist with support planning as necessary and also link people into universal and voluntary services. You will also assist colleagues within other areas of the services as necessary and support with complex assessments, mental capacity and best interest assessments.
The ability to develop, maintain and improve relationships with service users and carers is essential in this role as well as the attributes required to participate in inter-disciplinary team working.
