Pay rate £30 per hour

We are recruiting for experienced Social Worker to work in a Hospital Team in Manchester City Council. You will need experience in a similar Local Authority role.
 
Duties include working with the Discharge Team on discharge planning, Care Act 2014 Strengths Based Assessments for "Pathway 3"/"Pathway 2" care home placements, attending Best Interest Meetings for discharge decision-making, Safeguarding cases as they relate to Manchester Foundation Trust, Making Safeguarding Personal discussions, Neighbourhood Apartment referrals, and Crisis Clean referrals. We have a Hybrid working arrangement- working on site on Duty days, and from home the remainder of the week. We also work weekends 1 in 7 weeks in conjunction with Hospital Discharge to Assess team in Gorton.
