Adult Social Worker

Working Hours: 35 Hours per week
£30 per hour
Contract Type: Temporary up to 3 months

• Social Work Qualification and registration with Social Work England.
• Medical Clearance - Pre-placement health assessment (PPHA)
• Car Documentation -.
• DBS (Disclosure and Barring Service) Clearance -

The hospital discharge to assess team support citizens through the hospital discharge processes. Our input, if required, starts in hospital and continues into the community where a full assessment will take place in an appropriate setting.
You must be willing to be flexible and able to work at a fast pace. You will need to be a confident practitioner with a good understanding of the Social work role to ensure effective multi-disciplinary working.
We work flexibly throughout the whole service, this includes; D2A, Crisis response, intermediate care and the hospital. Social workers will work within the multi-disciplinary team both in and out of the hospital to ensure a holistic approach to assessment.
