Pay rate up to £30 per hour.
Bolton Require a SW with CMHT experience, ideally candidates should have Paris or Liquid Logic experience too. 
This is an exciting opportunity for social workers to join the Community Mental Health Team in Bolton that provide a comprehensive multi-disciplinary service. The Bolton CMHT is an integrated health and social care service that strives to deliver a recovery focused, trauma informed, needs based service for people with severe and enduring mental health conditions. We are a friendly and supportive MDT team who value working with people from diverse backgrounds and cultures. The Functional CMHT’s are part of Bolton Division of Greater Manchester Mental Health NHS Foundation Trust and provide a comprehensive multi-disciplinary service to people aged 18 years and above. We therefore welcome flexible and creative working to meet the needs of the people we support. As a Social Worker within in the Bolton CMHT you will assess service users using identified assessments, risk assessments and undertake the role of Care Coordinator in line with CPA process. You will have a defined complex caseload which could consist of people who are on the Assertive Outreach pathway and will be expected to work proactively and flexibly within a multi-disciplinary team model.
3 years reference, gaps in employment history checked, any relevant quals as per JDPS
