Pertemps Social Care are currently recruiting experienced Qualified Social Workers in Bromley to join the Discharge team! 
There is plenty of hybrid working available. 

Job Description:
• Complete Care Act Assessment and Mental Capacity Assessments
• Attend daily conference call in the mornings at 08:30
• Attend once a week multi-disciplinary team meeting on Tuesdays
• Support the rehab ward team with any client who requires social service input
• Complete joint home visit with rehab therapists when needed
• Liaise with other multi agencies
• Complete Safeguarding/ Dols
•Complete Placement, Extra Care Housing (ECH)
• Promote direct payment
• Present cases to Practice Review Group
• Knowledge regarding community resources
• Attend face to face meeting

Requirements:

• Hold a Social Work England registration 
• Eligibility to work in the UK
• Full UK Driver’s Licence
• Post-qualifying experience 
• Degree/equivalent in Social Work
