Are you a qualified Adults Social Worker seeking a role that makes a real difference?
Pertemps are working with a supportive and forward-thinking Adults Social Care Transition Team working across Lancashire, to find an experienced Adults Social Worker who can work collaboratively with healthcare professionals.
 
Adults Social Worker Role Overview:
As part of this innovative service, you will play a crucial role in providing a strengths based, outcome focussed assessment service to all adults (with the exception of adult mental health) across community and hospital settings.

Adult Social Worker Key Responsibilities:  

• To contribute to the delivery of strengths based social care service as outlined in national legislation and guidance and in line with county policies and procedures 

• To contribute to the provision of high quality assessments and support plans

• To work collaboratively with health colleagues to ensure safe and timely discharges from hospital the most appropriate service and setting for the individual that actively supports their choice and independence 

• To promote the creative and flexible use of personal budgets to appropriately meet the needs of customers and carers 

• To communicate effectively with customers, carers, other agencies and professionals to support the delivery of a co-ordinated response to customer and carer needs 

• To effectively prioritise and manage your caseload 

• To keep accurate records in relation to the contact and work undertaken, using electronic record systems (Liquid Logic) and other relevant ICT systems

Candidate Requirements for the Adults Social Worker role:

• A recognised social work qualification (e.g., Degree in Social Work, DipSW, or equivalent)
• At least 3 years’ post-qualifying experience 
• Current registration with Social Work England
• Experience in adult social care
• Strong communication and interpersonal skills, with the ability to work effectively in a team
• Knowledge of relevant legislation, policies, and procedures related to adult social care
