Adults and Integrated Care Service
Rate: up to £16.25 per hour

We require a Social Care Practitioner with a wealth of experience working in Adults w Learning Disabilities Team.
 
The Adults and Integrated Care Service is responsible for a range of services and activities that support people with social care and health needs, including carers.  The approach used by the service puts more focus on supporting people to remain independent and to live better lives.  Our Let’s Talk Together approach, based on 3 conversations, puts the person at the centre of their decision making, as experts in their own lives.  Our staff enable people to explore the networks around them that can support:

  • Maintain and develop their independence
  • Prevent, reduce and delay needs
  • Connect them to their community
  • Build on their strengths and assets
 
The role of Social Care Practitioner will require collaborative working with Social Workers and other professionals and colleagues, delivering bespoke intervention work across the South Tyneside Borough.
 
Key Tasks of the Post: 
  • To use active listening skills, observation and communication to build relationships with adults, their families and informal network supports and provide advice and guidance to help support them to navigate their way through systems.
  • To safeguard individuals in a way that supports them in making choices and having control in how they choose to live their lives. Ensuring that the ‘Making Safeguarding Personal’ ethos is at the heart of all practice.
  • To work directly with the person to support them to undertake an assessment of their needs using our Let’s Talk Together approach and enabling them to identify the outcomes they wish to achieve, whilst maximising their independence and choice. This will involve drawing on the support of families and informal carers and adopting a strengths-based approach.
  • To understand the roles and accountabilities of other agencies and professionals in order to refer, liaise, consult and work effectively in order to meet identified needs.
  • To work with the person or carer to develop and agree a support plan to achieve the identified outcomes to meet assessed eligible need, within resource allocation.
  • Under supervision, ensure the proactive and appropriate positive management of risk with adults, their families and carers and report accurate information and observations to line management for their consideration and further instructions.
  • To keep appropriate, accurate and timely records that describe and support analysis of the adults’ experience, clearly identifying the adults voice and demonstrate any decision-making rationale, whilst maintaining a high level of data quality.
  • To prepare and present reports of a statutory and non-statutory nature.
