Qualified Social Worker – Intermediate Care (Lambeth Rehab & Reablement Team) 
Rate: £30 per hour 
Contract: 6 months 
Hours: 35 hours per week, Monday–Friday, 9am–5pm 
Work Pattern: 3 days in office, 2 days home working 
About the Role 
We are seeking an experienced Qualified Social Worker (minimum 2 years post‑qualifying experience) to join the Intermediate Care Lambeth Rehab & Reablement Team. This is a unique and exciting opportunity to work within an integrated multidisciplinary service, supporting adults who are receiving therapy input and time‑limited reablement support. 
This role contributes directly to a project focused on data collection and analysis, specifically around reviews carried out by the Reablement Team for residents with existing care packages who are also receiving therapy input through our integrated partnership with GSTT, as well as MDT input from other allied health professionals.  
The role will also include assessing residents with new Reablement care packages following: 
Hospital discharge 
Input from our Urgent Response At Home Team 
Referrals from our front door team (ICAS) 
The purpose of these reviews is to assess progress, determine ongoing needs, and establish whether packages of care can be reduced, or ended after therapy intervention. 
Visits can be completed during office days, with workers expected to be based in the office between visits.
