£214.13 per day  
We are seeking a committed and experienced individual to join us as Social Worker in our Preparing for Adulthood Team. This is an agency role, and we are looking for a qualified social worker with experience of working with young people transitioning to adult social care.  You will be working with Young Adults with various learning disabilities and impairments.  
You will work closely with colleagues in other departments, and agencies to meet the care and support needs, and outcomes for our young adults. 
You will become part of a stable, award-winning team, where we provide the opportunity for you to grow and develop and join a very innovative and supportive service.  
You will need to be an excellent communicator and able to work alongside colleagues in Children Services, Health, Education and Voluntary organisations.  
Job purpose 
To provide high quality, professional service to all transitioning young adults who require care support. To carry out strength-based assessments compliant to Care Act 2014 
To use skills and professional knowledge to ensure people remain safe and well in our communities.
