Adult Social Worker, LB of Ealing  
Assessment and reassessment of social care needs adopting strength based approach, reviewing complex care needs of residents, conducting Mental Capacity Assessments and best interest meetings, managing safeguarding adults cases in line with PAN London policies and procedures, managing hospital discharge cases, attending multi-disciplinary meetings to discuss safe and timely discharge of cases, working with multi-disciplinary professionals: GPs, nurses, physio-therapists, Occupational therapists, Housing, Fire Force staff, Voluntary Organisations, Private Organisations etc. 
£32 per hour.
