Pay rate to £40 per hour
Contract role, Supported Living Review
Pertemps are recruiting for experienced Adult’s Social Workers in North London.
This to work within a Supported Living Review function.
The service aims to
- Ensure the delivery of least restrictive and most cost-effective support models at a scheme level, enabling individuals to achieve their outcomes.
- Review the quality and affordability of accommodation.
- Ensure the delivery of quality care and support at a scheme level.
- performing reviews to ensure that assessments and support plans accurately represent the support outlined by the service and meet the individuals' outcomes.
- translating review information into structured documentation to enable the calculation of proportionate support.
