Frailty and Anticipatory Care (PROFACCT) Social Worker, Islington 
 £32.52 per hour 
 We are seeking a dynamic and committed Social Worker to join the Proactive Frailty and Complex Care Team (PROFACCT) – a collaborative service between Islington Council and Whittington Health. This innovative team focuses on anticipatory care, working with adults living with frailty and complex needs to reduce hospital admissions and promote independence.
