Mental Health Discharge Liaison Social Worker, Haringey  
 The role will involve conducting assessments, developing support plans, and ensuring the right services and resources are available for individuals' needs. This requires expertise in mental health legislation and a strong understanding of the social care system. You'll also collaborate closely with other healthcare professionals and agencies to provide the best possible care and support. The role will require attendance at formulation meetings, close working with our Mental Health social care locality social worker, regarding clients open to services and readmitted to hospital 
Pay rate – 32.52 per hour
