Social Worker, Adults Hospital Team, Kirklees 
We are looking to fill two vacancies within the hospital team. 
with prior experience working in a hospital setting. 
Role Overview: 
The social workers will be responsible for: 

• Assessing service users in hospital under the Care Act 

• Coordinating care and support plans to enable timely discharge from acute hospital settings 

• Conducting Mental Capacity Act assessments and Best Interests decisions, where appropriate 

• Undertaking post-discharge reviews, including attendance at Decision Support Tool (DST) meetings 

• Managing Adult Safeguarding cases as required 

£35ph
