 Pay - £33.20ph  
We now have the following Agency worker posts to fill on the Hospital Social Work Team as soon as possible. These are filling positions supporting discharge to complete assessments in pathway 1,2 and 3 under the Discharge to Assess model (D2A). The is a hybrid role combining working from home, carrying out care home visits and working from the office.
