We are looking for an Experienced Social Worker to work within our Busy Hospital Team within the Adult Social Care Team at Bracknell Forest.  
The person will need to have good knowledge of Hospital Discharges D2A experience, Ability to complete Mental Capacity Assessments, Safeguarding experience.  
Pay - £31.92 per hour
