Southwark – Adults Provider Services
£24.54 per hour (Umbrella)
Full Time - 36.5 hours 

Pertemps are recruiting for a passionate and dedicated SEND Progression Mentor to join Adults Provider Services at Southwark Resource Centre, supporting young people with SEND to achieve their full potential.

The Role As a SEND Progression Mentor, you will support young people with learning disabilities, autism, sensory needs and physical disabilities to achieve their Education, Health and Care Plan (EHCP) outcomes.

You will deliver outcome-focused, strength-based and time-limited interventions within the community, at the Southwark Resource Centre or other agreed locations, in line with EHCP outcomes.
This is a highly person-centred role requiring a proactive, compassionate and structured approach to supporting independence, inclusion and wellbeing.

Key Responsibilities
  • Provide person-centred support for individuals with complex needs
  • Develop communication skills and promote participation in tailored activities
  • Act as keyworker, identifying and facilitating meaningful EHCP outcomes
  • Plan and deliver interventions to maximise independence, resilience and social inclusion
  • Connect individuals to local networks, community and voluntary sector services
  • Identify available assets, support networks and accessible local resources
  • Provide advice and information to individuals and their support networks
  • Complete risk assessments and implement safe support plans
Experience
  • Experience supporting people with disabilities in a paid or voluntary capacity
  • Experience developing outcome-focused, person-centred support plans
  • Experience supporting individuals with goal setting and progression planning
Skills and Competencies
  • Ability to prepare reports and maintain accurate records
  • Strong communication skills with individuals, families, carers and professionals
  • Experience supporting with physical care needs including mobility, transfers and personal care, maintaining dignity at all times
Special Conditions
  • Enhanced DBS clearance required
  • Must be physically able to meet the demands of the role including moving and handling
  • Flexible working may be required, including evenings and weekends
