About the Role: An excellent opportunity has become available for an experienced
Occupational Therapist to join the
Adults Community Team in Monmouthshire.
This role focuses on delivering high-quality, person-centred Occupational Therapy interventions to help individuals maintain independence, avoid unnecessary hospital admissions, and access the right care, equipment, and home adaptations.
Key Responsibilities:- Deliver Occupational Therapy interventions within service users’ homes and community settings
- Complete comprehensive assessments and develop tailored intervention plans for individuals with long-term or complex needs
- Support admission avoidance through timely and effective therapeutic input
- Recommend and arrange assistive equipment and home adaptations, including Disabled Facilities Grants (DFGs)
- Conduct housing assessments and contribute to manual handling and reablement plans
- Participate in a weekly duty rota, responding to queries and urgent referrals
- Act as a Trusted Assessor to set up and review care packages
- Manage a varied caseload efficiently, demonstrating excellent organisational and clinical reasoning skills
Key Relationships: You will work closely with:
- Community-based service users and their families/carers
- Multidisciplinary teams within Chepstow Community Hospital
- Social care teams, housing departments, and external agencies
- Duty team colleagues and wider Occupational Therapy services
- Care providers and equipment suppliers
Requirements:- Qualified Occupational Therapist
- Registered with the Health and Care Professions Council (HCPC)
- Experience working within community or hospital-based adult services
- Strong assessment and communication skills
- Flexible, motivated, and able to work independently and as part of a team
- Welsh language skills are desirable but not essential
