Occupational Therapist – Adults Services
📍 Location: Monmouthshire (based at Chepstow Community Hospital)
💷 Pay Rate: £32–£36 per hour
🕒 Working Hours: 9:00am – 5:00pm, Monday to Friday
🚗 Working Pattern: Community-based with travel to service users’ homes
📅 Contract Type: Temporary
About the Role:
An excellent opportunity has become available for an experienced Occupational Therapist to join the Adults Community Team in Monmouthshire.
This role focuses on delivering high-quality, person-centred Occupational Therapy interventions to help individuals maintain independence, avoid unnecessary hospital admissions, and access the right care, equipment, and home adaptations.
Key Responsibilities:
  • Deliver Occupational Therapy interventions within service users’ homes and community settings
  • Complete comprehensive assessments and develop tailored intervention plans for individuals with long-term or complex needs
  • Support admission avoidance through timely and effective therapeutic input
  • Recommend and arrange assistive equipment and home adaptations, including Disabled Facilities Grants (DFGs)
  • Conduct housing assessments and contribute to manual handling and reablement plans
  • Participate in a weekly duty rota, responding to queries and urgent referrals
  • Act as a Trusted Assessor to set up and review care packages
  • Manage a varied caseload efficiently, demonstrating excellent organisational and clinical reasoning skills
Key Relationships:
You will work closely with:
  • Community-based service users and their families/carers
  • Multidisciplinary teams within Chepstow Community Hospital
  • Social care teams, housing departments, and external agencies
  • Duty team colleagues and wider Occupational Therapy services
  • Care providers and equipment suppliers
Requirements:
  • Qualified Occupational Therapist
  • Registered with the Health and Care Professions Council (HCPC)
  • Experience working within community or hospital-based adult services
  • Strong assessment and communication skills
  • Flexible, motivated, and able to work independently and as part of a team
  • Welsh language skills are desirable but not essential
