Pertemps Social Care are currently recruiting for an experienced
Community Occupational Therapist to join Lewisham Council’s Intermediate Care Service. This is a fantastic opportunity offering a competitive hourly rate and the chance to work within a dynamic, multi-disciplinary team.
Main Purpose of the Role: To provide a responsive and high-quality Intermediate Care Occupational Therapy service for adults with physical disabilities and complex needs (including mental health conditions) across the borough of Lewisham. You will work as a vital part of a multi-disciplinary team supporting residents in their homes and within bed-based Intermediate Care settings.
Key Responsibilities:- Conduct Occupational Therapy assessments in people’s homes or care settings to evaluate functional abilities.
- Complete Community Care Assessments and contribute to personalised support planning with the wider team.
- Advise and support individuals and their carers to work toward rehabilitation goals.
- Recommend and arrange provision of equipment and minor adaptations to support independence.
- Teach safe use of equipment and alternative techniques to clients and carers.
- Liaise with residential and health care staff to ensure ongoing support for clients.
- Refer individuals to internal and external agencies where additional support is needed.
- Promote rehabilitation outside the home through links with Day Centres and community initiatives.
Requirements:- HCPC-registered Occupational Therapist
- Previous experience in community OT or rehabilitation/intermediate care
- Strong assessment and care planning skills
- Ability to work autonomously and as part of a multi-disciplinary team
