Job Description

Overall Purpose: To provide a client centered approach to assisting people with complex disabilities to achieve their maximum level of functioning and independence in their own homes. Occupational Therapy interventions focus on adapting the environment, modifying the task, teaching skills and educating clients and families in order to eliminate environmental barriers to independence.
 
Principal Accountabilities
1.
To assess and provide appropriate client centered Occupational Therapy intervention assisting people with disabilities to achieve their maximum level of functional independence.
2.
To supervise and facilitate the development of The Disability Resource Team Staff including Occupational Therapy Assistants, Occupational Therapists (PT 31), Sensory, Duty, Joiners, and Blue Badge staff.
3.
To provide professional assessment, advice and training to service users, carers, relatives and professionals with core and specialist equipment.
4.
To assess complex clients and make recommendations for major and minor housing adaptations that maximizes resources.
5.
To assess and provide specialist equipment to meet complex needs in the most cost effective way.
6.
To assess and provide moving and handling provision including prescribing and demonstration of equipment to meet complex service user’s needs.
7.
To expedite and facilitate hospital discharges.
8.
To assess and provide reablement interventions to regain functional independence.
9
To carry out independent mobility assessments to facilitate the provision of blue badges.
10.
To provide specialist Occupational Therapy assessments in a Prison Setting and in response to requests from care management for specific assessments.
11.
To support continuing professional development of the team.
