Social Worker, £35ph, Reablement, Slough
Pertemps are recruiting for a qualified and experienced Reablement Social Worker to support the transition of cases to long term packages of care through to locality teams The successful candidate will work within a multi-disciplinary team and support Occupational Therapists and Physiotherapists with Care assessments and Mental Capacity related tasks.

This position is full time Mon-Fri, with hybrid working – 2 days a week office based and 3 days home working.
