Pembrokeshire County Council is seeking an experienced and motivated Social Worker to join the Assessment Team based in Haverfordwest. This is a fantastic opportunity to contribute to vital frontline services and support positive outcomes for individuals and families across the county.
Key Responsibilities:
- Undertake comprehensive assessments of individuals and families in need
- Develop, review, and implement care and support plans in line with legislation and best practice
- Work collaboratively with multi-agency teams to safeguard and promote wellbeing
- Maintain accurate and timely case records and documentation
- Ensure all work is compliant with relevant legislation, policies, and procedures
Essential Requirements:
- Registered with Social Care Wales
- Enhanced DBS (Disclosure and Barring Service) clearance
- 5 years of referenceable employment/work history
- Strong assessment and case management skills
- Ability to work under pressure and manage complex caseloads
- Excellent communication and interpersonal skills
