Are you an experienced Team Manager with a background in Adults Social Care and hospital discharge? Ready to take on a vital leadership role that directly supports safe and person-centred care for adults leaving hospital? Pertemps Professional Recruitment is seeking a skilled and motivated Qualified Social Work Team Manager to lead a dedicated Hospital Discharge Team in West Yorkshire. You’ll be at the heart of operations, driving quality practice, managing complex casework, and supporting your team through high-pressure frontline work.

🔍 Your Role Will Include:
  • Leading and supporting a team of Social Workers and practitioners delivering hospital discharge planning and safeguarding work.
  • Overseeing Care Act assessments, capacity decisions, and case management.
  • Managing a duty function within the team and ensuring timely, effective responses to safeguarding concerns.
  • Working closely with NHS, health professionals, and partner agencies to ensure seamless multi-agency working.
  • Ensuring your team’s compliance with relevant legislation including the Care Act 2014, Mental Capacity Act, Human Rights Act, and Mental Health Act.
  • Maintaining a strong presence across hospital sites and local offices to support staff and ensure operational excellence.
What We're Looking For:
  • Qualified Social Worker, registered with Social Work England.
  • Proven leadership/management experience in adults' services—especially within hospital discharge or safeguarding teams.
  • Excellent understanding of current adult social care legislation and statutory responsibilities.
  • Strong people management and supervision skills, with the ability to motivate and support team members.
  • Confidence managing complex cases and risk in a fast-paced environment.
  • A commitment to high-quality, person-centred care and positive outcomes for vulnerable adults.

  • This role is fully office-based, with regular attendance required at:
    • Calderdale Royal Hospital
    • Huddersfield Royal Infirmary
    • Princess Buildings, Halifax
